Pyogenic Granulomas develop as benign vascular tumors with rapid vascular growth. Silver nitrate is the first line of treatment. This patient developed a Pyogenic Granuloma after pinching their hand while cutting. After multiple rounds of silver nitrate treatment, surgical excision was recommended. Therapy consisted of wound care, range of motion exercises, desensitization, and scar management.

Due to the intricate repairs at this juncture, progression was done slowly and the patient donned a DBO for 8 weeks. The patient began with active and passive digit flexion. At week 6, focus was to progress digit extension, wrist extension, and then composite wrist and digit extension between weeks 6-8. The DBO was discharged at 8 weeks per physician order.

This patient presents with pain over the radial aspect of their thumb and wrist. De Quervain’s Tenosynovitis, sometimes known as “Mommy Thumb” can occur with new mothers. Repetitive motions such a as holding, lifting, feeding, and unbuckling car seats can lead to symptoms in new mothers. These motions may lead to inflammation of the two tendon sheaths in the first extensor compartment. The tendons sheaths of the APL and EPB become irritated.

Stage I consisted of utilizing a Hunter rod to create a pseudosheath for a FDP graft in stage II. Stage II consisted of an autograft utilizing the uninjured hand Palmaris Longus tendon. The graft was carried out utilizing a pulvertaft weave. Treatment began with a dorsal block orthosis and early active motion. Swipe ➡️

This patient sustained a small finger proximal phalanx fracture while playing basketball. He underwent percutaneous pinning of proximal phalanx. He began therapy at 3 weeks and will have the K-wires removed at 6 weeks post-op. A hand based ulnar gutter orthosis was fabricated to protect the pin sites and fracture. The surgeon determines if the patient is able to begin early motion with the K-wires or if they will only be placed in an orthosis. In the case we will begin gentle ROM exercises. The patient was provided with an extension orthosis to be worn for the first few days and then at night time. Active and Passive flexion/extension exercises were implemented throughout the day.

We are looking at intrinsic versus extrinsic tightness here. We passively flex the IP joints with the MCPs extended. This is compared to passively flexing the IP joints with the MCPs placed in flexion. Intrinsic muscle tightness is indicated if the IPs are tight with the MCPs in extension. Extrinsic tightness is indicated if the IPs are tight with the MCPs in flexion. Joint capsule tightness is indicated if the IPs are tight with both motions.
